What to Bring:  
1. DeKalb County Participation and Consent Form with Insurance info2.
    2. Please fill out ALL FORMS listed below and bring them with you to Registration;
3. Physical form completed (Dr. Griffin will perform physicals on April 20th @ 4pm for $25 if you need a physical)
   

4. 2011 Jaguar Fees (at least 1/2 payment[$200] is due at Registration:

March 23rd - $100

April 23rd/30th - $100

and next two installments are due: 

May 17th - $100

June 1st - $100
    5. $10.00 - Locker Rental Fee

For the 2011 football season, the non-refundable fees are: $400 per player

  • 2 Practice T-Shirt, 2 Practice Shorts

  • Custom Practice Jersey (With Players Name)

  • Sweater Vest

  • 2 Compression Shirts (1-Long Sleeve/ 1-Short Sleeve)

  • Nylon Sweat Suit

  • Banquet Activities

  • Pre-Game Meals
Fee-Raisers
Fee-Raisers are in place to assist you in paying your fees. Opportunities to participate in fee-raisers will be announced once available.
Sponsorship
Sponsorship is available immediately and involves you soliciting donations for the Stephenson Football Program from corporate and /or personal sponsors. Donations made to Stephenson Football will be credited to your balance at a rate of 50%. For example, if you solicit and receive a check for $500 from a sponsor, payable to Stephenson Jaguar Touchdown Club, 50% or $250 will be credited to your fee balance. The sponsor must write the player’s name in the memo section of the check.
